Sample Teacher Pre-Observation Form

CTA Family, I apologize for the delay, but as promised, I have attached a copy of pre-observation form completed by a teacher.  I know it it not exactly the same as ours, but since our observations are so similar to the teachers', I figured that it would not hurt to see what they do.
